HERSHEY PARK TRIP
Dear Middle School Band Students and Parents,
Hayfield Middle School bands will going on a field trip this year to Hershey Park in Hershey Pennsylvania on
Friday, May 24, 2019.
The groups will perform for nationally recognized judges, and then spend the day in the park. The bands will leave at approximately 5:30 a.m. and will return at approximately 11:00 p.m. that evening.
The cost of the trip will be about $155.00 per student. Accounts may be paid at one time, or in two installments.
The first installment of $85.00 and all forms are due in February 14, 2019.
The second installment of $70.00 is due March 8, 2019
Please make checks or money orders payable to Hayfield Secondary School.
Cash will not be accepted.
We will ask for the following forms:
-Parental Authorization and Acknowledgement of Risk for Field Trip
-Field Trip Luggage Search Consent
-Emergency Care Form
-Authorization for Medication (fill out ONLY if applicable)
A number of chaperones will be needed for the trip. We will need a minimum of one chaperone for every eight students.
The fee for each chaperone attending will be $100.
The 1st payment of $50 is due on February 14, 2019.
There is limited bus space on this trip, so chaperones will be accepted on a first come, first served basis, and are expected to attend an informational meeting at Hayfield on Wednesday, May 10 at 6:30pm in the middle school band room. A detailed itinerary, with contact information and trip specifics will be sent home in early May, and will be available on the band website.
